Time                :  6.00 pm to 11.00 pm

Venue              :  Vanda Room, Singapore Polytechnic

                           500 Dover Road, via Gate 3 – Staff Link Road / Besides Telecom Tower

Admission       :  $50 per person

 a)                  Complete and email your particulars to us by using the registration form below.

 b)                  Please send a cheque for $50 payable to “Tay Swee Kuan” as admission charge after emailing your registration.  This will serve as confirmation of your participation.  (Tay Swee Kuan is the Treasurer of the Organizing Committee of the DSTS Grand Reunion Dinner Party 2004 and also the owner of Foodhaven Restaurant at Singapore Polytechnic.)

For those commuting by MRT, walk  towards the Telecom Tower as your direct bearing.
